Man, Tokyo is hard, because they are so hard on the gaijin (foreigners). I looked and looked around when I was there, and found, one night, one time, a t-girl in the area called Shinjukuni-chome, outiside the Shinjuku station and around the Golden Gai area. The t-girl I met was not that great looking or really that interesting to me, but I went with it pretty much so I could say that I had a t-girl in Japan and Asia more than anything else. Hopefully you can find something better. -

There are several T-girl bars in Tokyo, mainly Philippina but others are also represented. Just do a search for new-half bar on any search engine. However, unless you can read or speak Japanese it will be very difficult to find these places. Your best bet would be if you have a Japanese friend who can show you around and get you into these places. There are several t-girl websites in Japanese and several escort sites.

I live in Tokyo so here's some infro that might help you:
1. As HairyChest mentioned Shinjuku ni-chome is the gay area of Tokyo. There are a few New-Half (the word the Japanese uses for T-girls) bars and hostess clubs. However they are difficult to enter unless you can speak Japanese or go with a Japanese person. They are also difficult to find. The one I went to was down some back alley street with no visible sign.
2. There are some Filipino and Thai T-girls that hang out in Roppongi. Roppongi is where most foreigners hang out and party. Lots of bars and clubs. The T-girls usually hang out at a bar called "Motown". They usually try to pick up some drunk unsuspecting gaijin for some extra cash. Some of them are hostesses, some freelance others who knows. Also they usually don't come out until after 11:00, sometimes as late as 1 or 2 am when some of the hostess bars close up. This is your best bet for some free T-girl ass.
3. In Kabukicho (a red light / entertaiment area) in Shinjuku, I've seen the occ. T-girl streetwalker. They are located past the Koma Stadium on the left hand side of a Police koban. That's right they ply their trade that close to the cops. There are some Japanese, chinese or filipino t-girls. Looks are about average. Cost about 15,000-20,000 yen plus love hotel charge. Watch out for the lowly pimp/Yakuza guys that hang around there.
4. Check out a free magazine called "Metropolis". Look in the "And others..." section. I've seen a T-girl ad there one time.
