Yes, they really do work and some can be very expensive. Years back my urologist prescribed one for me. This was a deluxe model ($600 CDN but covered by my health insurance) with an electric pump operated with a pushbutton, and had an automatic pressure relief, but many use a hand operated pump with a standard pressure guage attached in the vacuum line. There are dangers of causing a haematoma due to burst blood vessels because of too great a vacuum being pulled. The pharmacy which carried them had a loaner model so it could be tried for a month before committing to actually buying one.
The main drawback for me was excusing myself while I pumped myself up to operational rigour!
No you don't tie it off but there is uaually a set of different diameter rings included with them. Most models make use of an elastic (silicone) ring which is placed on the pump, then after desired erection is obtained the ring is rolled off onto the penis to keep it fully erect, the vacuum is released and the pump is removed. That constriction ring must not be left on for too long (mine suggested not more than 40 minutes), else you might end up with a black, rotting dick!

That is why I mentioned avoiding the cheapo units sold by novelty stores, porn sites etc. because they do not have any built in safety mechanisms. Using a hand operated pump with no pressure guage or safety pressure relief it would be very easy to draw enough vacuum to easily burst a blood vessel or tear other internal tissues and do irreparable damage. Also disregarding the need to remove the constriction ring after 30-40 minutes can also cause permanent damage.
