I'm pretty sure that's a real girl with a penis poorly cropped in with Photoshop. Look at the thin lines where the penis is attached to the body.

I’d also call attention to the blurred area (just below the patch of hair) which bisects that thin line you've pointed to. A little more blurring of that hard line might have made it look a little more realistic!
